Parts A and B of the BSA Annual Health and Medical Record are required of all Scouts and adults participating in any outdoor activity with our troop. They can be filled in online from the web link above and printed out to give to Mr. Carey, the Assistant Scoutmaster (ASM) for Outdoor Activities. For longer outings or for summer camp, the Troop must have not only Part A and Part B on file, but also Part C, which requires a physician's signature.
